29 CFR 1910.138(a):General requirements. Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes. a)On or about December 8, 2025, the employer failed to ensure that employees used appropriate hand protection while performing propane tank change procedures. A temporary employee sustained an injury while changing a propane tank on a forklift without wearing the designated orange rubber gloves and being sprayed with liquid propane. As a result, employees were exposed to chemical burns, cryogenic burns.

29 CFR 1910.178(a)(6):The user shall see that all nameplates and markings are in place and are maintained in a legible condition. a)On or about December 9, 2025, the employer failed to ensure that all control markings on a gas-powered forklift were in place and maintained in a legible condition. Employees operating and servicing the forklift in the material handling area were using to unlabeled controls, which prevented clear identification of each lever's function. This condition created a risk of incorrect operation, including unintended movement or activation of forklift components. As a result, employees were exposed to struck-by and caught-in hazards associated with the use of forklifts lacking proper control identification.
