29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  On or about December 9, 2025, employees were exposed to physical hazards through the operation of a powered industrial truck, and the employer did not provide the required forklift operator training. This training would include:  a. Formal instruction (lecture, discussion, interactive computer learning, video tape, written material), b. Practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and c. Evaluation of the operator's performance in the workplace.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about December 9, 2025, employee operating the Winntec Y461555 50 ton heavy duty Jack Press was exposed to potential flying debris, shrapnel, or part ejections while performing pressure testing on the air bottle neck jacks and the employer did not provide one or more methods of machine guarding.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  On or about December 9, 2025, employees working near the Winntec Jack Press were exposed to electrical and fire hazards because a box fan was plugged into an extension cord and the extension cord was plugged into a relocatable power strip.
