1910.147(c)(7)(i)(A) Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolattion a)On or about December 3, 2025, the employer did not fully document in the authorized employee training the means and methods for visually confirming that the exhaust fan and the connected belt and pulley that had been locked out at the electrical energy source had ceased all motion prior to removal of the guard that was located in front of the exhaust fan to prevent employee access to the exhaust fan and the connected belt and pulley. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Exhaust fan - On or about December 3, 2025, employees were exposed to machine hazards associated with moving parts while performing maintenance and servicing tasks on exhaust fan. The employer did not implement energy control application steps as the exhaust fan was not properly deenergized to perform maintenance on exhaust fan [per the 1910.147(d)(5) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) Machine Room - On or about December 3, 2025, the employer did not provide effective means or equipment to isolate, secure or block hazardous energy sources, such as block/pin/chain for the fan blade of the exhaust fan. Employees were exposed to kinetic energy from the fan blades and fan belt. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
