29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a)Manufacturing Systems, Inc. at 300 Industrial Street, Belle Fourche, SD 57717: On or about and at times prior to December 9, 2025, the employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury on or around industrial equipment located in the North Building and South Building Fabrication Areas. This condition exposed employees to struck-by, caught-in, and electrical hazards.

CNC Vertical Machining Center, Model Number: BMC-30, Serial Number: BC-8005079J was properly guarded. This condition exposed employees to caught-in and amputation hazards.
