29 CFR 1926.451(g)(2): The employer did not provide fall protection for employees erecting or dismantling supported scaffolds: a) An employee was not protected from falls when working from a metal platform supported by a Gehl rough terrain forklift. The platform was elevated approximately 18 to 19 ft above the dirt ground and construction materials. b) An employee was not protected from falls when working from the edge of the roof approximately 21 feet above the ground. The employee was installing the roof supports for the erection of pump jack scaffolding.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: (a) A metal platform elevated by a Gehl rough terrain forklift was moved horizontally while a worker was on the platform. The platform also did not have guardrails or other fall protection.

Workers were exposed to tools or equipment falling from approximately 18 to 20 ft.
