29 CFR 1910.30(a)(3)(iii): 29 CFR 1910.30(a)(3)(iii): The employer did not train each employee in the correct procedures for installing, inspecting, operating, maintaining, and disassembling the personal fall protection systems that the employee uses: a) Warehouse, Storage Racks - On or about December 9, 2025, an employee was required to operate a Hyster Order Picker, serial number 6A266078, to retrieve products. The employee had to retrieve orders that were approximately 20 feet above the ground from within a Koke MFG man basket attached to the forklift. The employee was not provided with fall protection training to wear a harness in the man basket.

29 CFR 1910.178(l)(1)(ii):The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck: (a) Worksite, Warehouse - On or about December 09, 2025, an employee was operating a Hyster Order Picker, serial number 6A266078, and a Crown Power Jack, serial number 6A249310, to retrieve products and move materials in the warehouse area. The employer did not ensure that all employees were provided with training to use the forklifts.
