29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Location: 6 Skyline Dr. Thiells, NY On or about: December 1, 2025 a) Three workers were at the bottom of a trench excavation approximately 15 feet in depth and approximately 30-35 feet in width without a stairway, ladder, ramp, or other safe means of egress so that no worker was required to travel 25 feet or less to exit. The only device present was a 6?foot A?frame step ladder, which did not reach from the trench bottom to the surface and did not provide safe egress. Workers exposed to trench excavation collapse due to lack of safe egress.

On or about: December 1, 2025 Location: 6 Skyline Dr. Thiells, NY 10984 a) Three workers were at the bottom of a trench excavation approximately 15 feet in depth and approximately 30-35 feet in width with the spoil pile placed along the edge of the excavation. The spoil pile was not placed at least two feet or more away from the excavation area. Workers exposed to overhead hazard.
