29 CFR  1910.218(a)(2): Inspection and maintenance. It shall be the responsibility of the employer to maintain all forge shop equipment in a condition which will insure continued safe operation.   a.) Channellock, Inc. 1306 South Main Street Meadville, PA 16335, Forge Shop, on or about 10/23/2025, and times prior thereto: Employee was operating the Ceco 1500-2 Re-Strike Hammer in the Forge Shop Area when the safety pull-back device failed. The rope on the Possons Pull-back safety device broke resulting in an employees hand getting crushed by the top die.   ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Channellock, Inc. 1306 South Main Street Meadville, PA 16335, Forge Shop, on or about 10/23/2025, and times prior thereto: Frequent inspections not conducted on key safety components including, but not limited to, the area where the rope passes through the eyelet at the Ceco 1500-2 Re-Strike Hammer in the Forge Shop Area.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
