29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a.) Channellock, Inc. 1306 South Main Street Meadville, PA 16335, on or about 01/08/2026 and times prior thereto: Employees were exposed to fall hazards associated with an unguarded 5 foot open Gas Mac Furnace pit in the Heat Treat Department . Guardrail was removed to work on the conveyor of the Gas Mac Furnace exposing employees to 5 foot fall hazards.   ABATEMENT CERTIFICATION REQUIRED:  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of the abatement.

29 CFR  1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a.) Channellock, Inc. 1306 South Main Street Meadville, PA 16335, Forge Shop, on or about 01/08/2026, and times prior thereto: Employees are exposed to hazards associated with the Terek TS20 scissor lift in the Compressor Room Area. Hazards include, but are not limited to, exposed electrical wires at the control box, hole on the side of control box, bent/damaged railing, and electrical tape on the electrical plug at the bottom of the lift. An inspection completed by a qualified person would identify the hazards with the lifts and the need to take the lifts out of service until repairs are made.   b.) Channellock, Inc. 1306 South Main Street Meadville, PA 16335, Forge Shop, on or about 01/08/2026, and times prior thereto: Employees are exposed to hazards associated with the JLG 2032E2 scissor lift in the Compressor Room Area. Hazards include, but are not limited to, multiple safety and directional labels on the JLG 2032E2 scissor lift were not legible to the operators of the lift.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Channellock, Inc. 1306 South Main Street Meadville, PA 16335, Forge Shop, on or about 01/08/2026 and times prior thereto: Employees are exposed to hazards associated with a bent prybar in the Heat Treat Department area while performing maintenance tasks.
