OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to Pinch Point/Caught between and amputation hazards associated with an unguarded foot control switch at the Pneumatic bench vise.     a.) Channellock, 1306 South Main Street Meadville, PA 16335, on or about 1/21/2026 and times prior thereto: Employees in the WB08 Finishing Area were exposed to hazards associated with an unguarded foot control pedal at the Pneumatic Heinrich Production bench straightening vise (serial # 09113). The foot pedal was not guarded which could allow unintentional activation of the machine from human input and/or other objects.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a.) Channellock, 1306 South Main Street Meadville, PA 16335, on or about 1/21/2026 and times prior thereto: Employees in the WB08 Finishing Area perform servicing and maintenance on equipment including, but not limited to, the Pneumatic Heinrich Production bench straightening vise (serial # 09113). The pneumatic vise was not deenergized and Lockout tagout was not applied while performing cleaning activities.   ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement

Channellock, 1306 South Main Street Meadville, PA 16335, on or about 02/09/2026 and times prior thereto: The area in front of 120/240V electrical panel #100-32 was obstructed by containers of pliers in the Machine Shop Department.  NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
