On or about December 10, 2025, at 3500 South Carrollton Avenue, New Orleans, LA 70118, employees were required to work in an automotive service facility where two designated exit doors (the southeast bay entrance and southwest bay entrance) were blocked by stored materials and equipment. During periods when the bay doors were closed-including before opening, after closing, and during cold weather-employees had limited or no access to an unobstructed exit route, exposing them to fire and entrapment hazards in the event of an emergency.  1.) South-east bay entrance 2.) South-west bay entrance
