29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)Independent Exterior and Repair, Inc. at 502 North Creek Drive, Rapid City, SD 57701: On or about and at times prior to December 10, 2025, the employer did not ensure that employees were protected from falls while conducting roofing operations on a multi-family residential structure. This condition exposed the employee to fall hazards greater than six feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladders side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)Independent Exterior and Repair, Inc. at 502 North Creek Drive, Rapid City, SD 57701: On or about and at times prior to December 10, 2025, the employer did not ensure that the Little Giant Velocity 17-foot aluminum multi-purpose ladder, Type IA that employees were using was extended at least three feet above the upper landing surface of the west roof eave of a multi-family residential structure.  This condition exposed employees to fall hazards.
