29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired Navy Research Laboratory, Area C: On or about December 10th, 2025, and at times prior thereto, employee(s) used a 9-foot A-frame step ladder to perform plumbing, piping and framing work. The A-frame ladder used did not have any manufacture data listed for safety requirements nor load limits for employees to identify during inspection and use of ladder. Employees were exposed to the potential of ladder overloading, mechanical failure and/or fall hazard(s).
