29 CFR  1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite.  Repair Hangar Bldg.801, Roof:  On or about December 10th, 2025, and at times prior thereto, the employer did not ensure a fire extinguisher was provided for employee use. Employee(s) utilized multiple flammable spray adhesives to conduct roofing work. This condition potentially exposed employee(s) to fire hazard(s).

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  Repair Hangar Bldg.801, Area B: On or about December 10th, 2025, and at times prior thereto, employee(s) used an X-Treme portable power distribution unit to supply power for the installation, but it did not have strain relief and had exposed the inner wiring. This condition potentially exposed employee(s) to electrical shock and/or fire hazards.

29 CFR  1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  Repair Hangar Bldg.801: On or about December 10th, 2025, and at times prior thereto, employee(s) were tasked to conduct roofing work. The employer did not ensure the scaffold, erected to a height of 65feet, had been inspected for visible defects by a competent person prior to use. This condition potentially exposed employee(s) to fall, Struck-by and Crushing Hazards.

29 CFR  1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface.  Repair Hangar Bldg.801, Roof: On or about December 10th, 2025, and at times prior thereto, the employer exposed employees to fall hazards of greater than 10 feet, in that, the warning line system was not maintained to prevent sagging below the minimum height of 34 inches from the walking/working surface. This condition potentially exposed employee(s) to fall hazard(s).

29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  Repair Hangar Bldg.801, Area A: On or about December 10th, 2025, and at times prior thereto, employee(s) used a 9-foot A-frame step ladder to perform plumbing, piping and framing work. The A-frame ladder used did not have any manufacture data listed for safety requirements nor load limits for employees to identify during inspection and use of ladder. Employees were exposed to the potential of ladder overloading, mechanical failure and/or fall hazard(s).
