On or about December 9, 2025, at an establishment located at 5800 West Drive in Manitowoc, Wisconsin:, in the Compounding Household Blending Department, near the storage tank 47 interface panel, the employer failed to ensure a junction box installed in a wet environment where water could enter or accumulate was enclosed and weatherproof.
