29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. On or about December 9, 2025, at an establishment located at 5800 West Drive, Manitowoc, Wisconsin: employees were exposed to hazards related to powered industrial vehicle operation while utilizing the Toyota sit down forklift 53 (Model: 8FBE20U, Serial: 11016) which had damage including, but not limited to radial cracking and tires that were pulling away from the rim.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures. Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations shall be installed so as to prevent moisture or water from entering and accumulating within the enclosures and shall be mounted so there is at least 6.35-mm (0.25-in.) airspace between the enclosure and the wall or other supporting surface. However, nonmetallic enclosures may be installed without the airspace on a concrete, masonry, tile, or similar surface. The enclosures shall be weatherproof in wet locations. On or about December 9, 2025, at an establishment located at 5800 West Drive in Manitowoc, Wisconsin:, in the Compounding Household Blending Department, near the storage tank 47 interface panel, the employer failed to ensure a junction box installed in a wet environment where water could enter or accumulate was enclosed and weatherproof.

29 CFR 1910.22(d)(1): Walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition; On or about December 9, 2025, at an establishment located at 5800 West Drive in Manitowoc, Wisconsin, in the Household Compounding Department, by the finished product tank, employees were exposed to slips, trips and falls when the employer failed to correct or guard an area near a floor drain on the main walkway. The epoxied surface had been broken or dislodged and no longer presented an even walking-working surface.
