OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were not protected from the hazard of being struck by or crushed by large quartz or stone slabs during unloading. Location: Fabricator site at 500 Lynnway, Revere, MA On or about 10/13/2025, employees were exposed to struck-by and crushing hazards while a slab was unsecured during unloading transition after transport restraints were loosened or removed and before the slab was secured by receiving equipment, including where adverse weather or wind conditions affected slab stability.
