29 CFR  1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  Facility-wide:  On or about December 4, 2025, when personal protective equipment was required, the employer did not provide written certification confirming that a comprehensive workplace hazard assessment had been conducted. Employees encountered splash and irritating mist hazards while handling and mixing substances identified as hazardous to eyes and skin. Products utilized included EasiSolv 701N, which contains 40% DPMA, designated as a Category 2 irritant, and EasiStrip SUPRA One Step Ink & Emulsion Remover, containing 6% Disulfonate and sulfuric acid, classified as a Category 1 reagent. Both products pose significant splash risks that may result in severe ocular injuries, burns, or skin irritation.

29 CFR  1910.132(f)(1)(iii): Employee(s) required to use personal protective equipment (PPE) by this section were not trained to know how to don, doff, adjust, and wear PPE:  mall Press Room - Power Wash Area:  On or about December 4, 2025, and some time thereafter, the employer did not ensure employees who were required to use personal protective equipment (PPE) were trained to know how to don, doff, adjust, and wear the PPE when hand protection and suitable eye or face protection that covers the eyes and face from splash hazards while exposed to hazardous chemicals, acids, and caustic liquids. Employees poured and soaked print screens in a tank with diluted ink and emulsion remover, then transferred screens for power washing and scrubbing with EasiSolv 701N Screen Wash & Stain Remover. Despite risks of chemical burns and eye damage, required PPE like goggles and face shields were not provided. Chemicals used, including EasiSolv 701 (40% DPMA, Category 2 irritant) and EasiStrip SUPRA One Step Ink & Emulsion Remover (6% Disulfonate, sulfuric acid, Category 1 reagent), posed significant splash hazards that could result in severe eye injuries, burns, or skin irritation.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   Small Press Room - Power Wash Area:  On or around December 4, 2025, employees were exposed to corrosive substances, including a 6% Disulfonate and sulfuric acid solution, without access to appropriate emergency eyewash facilities in the immediate work area. This exposure occurred while personnel performed tasks involving splash hazards, such as transferring approximately two cups of EasiStrip SUPRA One Step Ink & Emulsion Remover from a 5-gallon container into a water-filled 55-gallon tank. The emulsion remover is classified as a hazardous corrosive material (Category 1), with the potential to cause severe eye damage or irritation.
