The employer did not ensure that the powered industrial trucks operated by employees in the workplace had all labels in place on the operator control levers. The powered industrial truck used was a SKY TRAK all terrain forklift.

Employees conducting residential framing work were observed using pneumatic nail guns to fasten lumber together without eye or face protection.

29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval:   At the residential worksite located at Lot 75 Acacia  Dr, Lake Lotawana, MO 64086:  Employees were exposed to fall hazards.  The employer modified the powered industrial truck, without manufacturer's written approval, when attaching a homemade platform which employees were working from while conducting residential framing activities.  Golden Hammer LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.602(c)(1)(ii), which was contained in OSHA inspection number 1644090, citation number 1, item number 2a, and was affirmed as a final order on 04-05-2023, with respect to a workplace located at 27430 E Oak View Drive, Lake Lotawana, Missouri.
