Condition observed on or about 12/11/25.

This after market plug was not listed, labeled or certified by the saw's manufacturer.

29 CFR  1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.   a- 1213 & 1214 Auburn Terrace, Manalapan NJ:  A worker was exposed to worksite fall hazards, approximately 13 feet 9 inches, when using a damaged aluminum extension ladder at the worksite.  The company worksite Foreman and this worker did not receive ladder hazard recognition and minimization training from the employer at the time of  and prior to the worksite inspection.  Condition observed on or about 12/11/25.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
