29 CFR  1926.451(g)(1)(vii):For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  A)1098 Woodbridge Center Dr, Memorial Gardens Tower B, Woodbridge, NJ 07095  Four employees were permitted to work from a job-made scaffold 14'3'' above the ground without any fall protection in place. 29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting  the ladder:  A) 1098 Woodbridge Center Dr, Memorial Gardens Tower B, Woodbridge, NJ 07095  The ladder being used to access the scaffold pick boards didn't extend 3ft above landing and wasn't secured at the top.  Hazard noted on or about 12/11/25

29 CFR 1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section (fall protection training) by preparing a written certification record which contained the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  A) 1098 Woodbridge Center Dr, Memorial Gardens Tower B, Woodbridge, NJ 07095  The employer did not ensure, through a written certification, that employees had been adequately trained in the hazards associated with falls and the proper ways and procedures to take in order to minimize the hazards.
