29 CFR 1910.147(d)(5)(i): All potentially hazardous stored or residual energy was not relieved, disconnected, restrained or otherwise rendered safe after the application of lockout or tagout devices to energy isolating devices: On or about December 12, 2025, the established energy control procedure for the two Grinder/Mixers (Weiler Grinding Machines) on Line 3 did not cover the means to protect employees from hazards caused by the residual energy of the rotating the Auger which would continue to rotate for approximately 35 seconds after the machine was turned off. Employees perform lockout on the machine approximately 4 times per day to disassemble and reassemble the machine for Quality Assurance checks and Sanitation.
