29 CFR  1910.179(h)(4):  Hooks did not meet the manufacturer's recommendations:  a) Integrity Granite, Shop: On or about December 10th, 2025, and at times prior, employees were exposed to struck by hazard while operating the Jet 1/2 ton overhead crane to lift and stage stone granite slabs. The hook being used did not contain a throat safety latch.  b) Integrity Granite, Shop: On or about December 10th, 2025, and at times prior, employees were exposed to struck by hazard while operating the Milwaukee overhead crane to lift and stage stone granite slabs. The hook being used did not contain a throat safety latch.

29 CFR  1910.179(j)(1)(i):Prior to initial use, all new and altered cranes were not inspected to insure compliance with the provisions of 29 CFR 1910.179(j).  a) Integrity Granite, Shop: On or about December 10th, 2025, and at times prior, employees were exposed to struck by hazards while operating overhead cranes including, but not limited to the Milwaukee 1 ton crane, to lift and stage stone granite sheets without having performed an initial inspection of the crane prior to use.

29 CFR  1910.305(j)(2)(v): A receptacle installed outdoors in a location protected from the weather or in other damp locations did not have an enclosure for the receptacle that was weatherproof when the receptacle was covered.  a) Integrity Granite, Shop: On or about December 10th, 2025, and at times prior, employees were exposed to electric shock hazards while wet cutting, grinding, and polishing stone granite slabs around exposed electrical junction boxes. Throughout the shop each junction box was exposed to water overspray from the wet methods utilized by the employer.

29 CFR  1910.1053(d)(1):The employer did not assess the exposure of each employee who was or may reasonably be expected to be exposed to respirable crystalline silica at or above the action level in accordance with either the performance option in paragraph (d)(2) or the scheduled monitoring option in paragraph (d)(3) of this section:  a) Integrity Granite, Shop: On or about December 10th, 2025, and at times prior, employees were exposed to airborne respirable crystalline silica while cutting, grinding, and polishing stone granite slabs. Full shift sampling was performed at the facility where one employee was exposed to respirable silica at 0.058 milligrams per cubic meter of air measured over 453 minutes. The remaining 27 minutes of sampling was calculated at a zero exposure. The Action Level (AL) for airborne respirable crystalline silica is 0.025 milligrams per cubic meter of air.
