29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Location: 133 Kent Avenue, Brooklyn, NY 11249 On or about: December 11, 2025 (a) The employer did not ensure that strain relief was provided to the flexible extension cord used to energize a hand held power tool; the extension cord with strain damage exposing the internal wire insulation was used to energize a DeWalt DWE402 angle grinder.
