Capacity, operation, and maintenance instruction plates, tags, or decals were not clearly marked on the attachment. WORKSHOP/290 PRATT STREET, LOWER LEVEL, SUITE 1108, MERIDEN, CT 06450: On/or about December 11, 2025, and prior to, the employer modified the use of the Caterpillar powered industrial truck (PIT) affecting capacity and safe operation without the manufacturers' prior written approval by use of the front-end boom mounted attachment to the forks of the (PIT). Capacity, operation, and maintenance instruction plates, tags, or decals were not clearly marked on the attachment.
