29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about December 11, 2025, Bull Construction Inc. employees were exposed to fall hazards while performing roofing work at heights greater than 25 feet without the use of an approved fall protection system. The employer failed to ensure employees were properly protected from falling to the ground below while working at the jobsite located at 18375 Chesterfield Airport Road in Chesterfield, Missouri.

The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards. On or about December 11, 2025, Bull Construction Inc. did not provide a training program for employees working at heights to recognize and minimize hazards associated with falls at a worksite located at 18375 Chesterfield Airport Road in Chesterfield, Missouri. The employees were wearing fall protection systems incorrectly and in a manner which increased the likelihood of serious injury.

On or about December 11, 2025, Bull Construction Inc. was operating a JCB Loadall, rough terrain forklift, model 512-56, without ensuring employees operating the powered industrial truck had received a combination of formal instruction, practical training, and evaluation of the operator's performance in accordance with the requirements set forth in 1910.178(l)(2)(ii).
