29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:   a) Job site, 8254-8256 Ohio River Boulevard, Etna, PA: On or about December 12, 2025 during residential construction activities, the employer did not ensure employees working on a roof used appropriate fall protection systems/devices, exposing them to fall hazards of up to approximately 12 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 8254-8256 Ohio River Boulevard, Etna, PA: On or about December 12, 2025 during residential construction activities, the employer did not ensure the ladder used to get on/off the porch roof was properly set up in that it only extended approximately 1 1/2 feet above the roof line.

29 CFR 1926.20(b)(2):  A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person:   a) Job site, 8254-8256 Ohio River Boulevard, Etna, PA: On or about December 12, 2025 during residential construction activities, the employer did not ensure a system was in use at the site to identify and correct safety and health issues. Deficiencies noted during the inspection included, but were not limited to, not using eye/face protection when cutting with power tools, simultaneously standing on two ladders side by side, not using fall protection when needed and not having a ladder extended at least 3 feet above the landing surface.
