29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) On or about December 11, 2025, at 7600 Northwest 82nd Place Medley, FL, an employee while retrieving boxes from a steel rack at approximately 10 feet above ground level with a lanyard was not attached to an anchor at the Crown Aerial Work Platform (AWP)Model Number SP3510-30, exposing him to a fall hazard.
