29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition. a)Warehouse - On or about December 12, 2025, in the narrow aisle of the cardboard box palletizing location, the employer did not ensure that walking?working surfaces were regularly inspected, maintained, and kept in a safe condition. The aisleway was too narrow and was not kept clear of materials, resulting in inadequate space for safe travel through the area. Employees working in the aisle were exposed to struck?by hazards due to the employer's failure to identify and correct the hazardous condition through regular inspection of walking?working surfaces.

29 CFR 1910.176(a): Permanent aisles or passageways were not appropriately marked: a) Warehouse - On or about December 12, 2025, at the cardboard box palletizing location, the employer did not ensure that adequate clearance was provided and maintained for forklift (Toyota Forklift Model #8FGU15 SN#64336) operations within a narrow aisle. Materials, equipment, and the structural layout restricted the available travel path, resulting in insufficient space for safe forklift maneuvering. Employees working in this area were exposed to crushing and struck?by hazards. As a result of these hazardous conditions, an employee was struck by a forklift and sustained serious injuries.

29 CFR 1910.178(n)(6): Industrial truck drivers were not required to look in the direction of, and keep a clear view of the path of travel: a) Warehouse - On or about December 12, 2025, in the cardboard box palletizing area, the employer allowed employees to walk and work adjacent to and in front of an operating powered industrial truck (Toyota Forklift Model #8FGU15, SN#64336) without ensuring that safe operating practices were followed. The forklift operator reversed the truck without maintaining a clear view in the direction of travel, looking only over the left shoulder and failing to verify that the travel path was free of employees on the right side. As a result, the forklift struck an employee, running over the employee's left leg and causing severe injuries.
