29 CFR 1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Site- Employees operating a JLG 1230ES scissor lift were not provided with operator and fall hazard training prior to operating the lift. On or about December 12, 2025, an employee fell approximately 5 feet from a scissor lift while trying to place a strap around an overhead pipe.
