29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a)666 Services/600 Federal Boulevard, Carteret, NJ 07008/Scan station #4/Employees who work on the scan stations are exposed to unexpected start-ups or release of stored energy as the employer did not have a safety program established to include energy control procedures, employee training and periodic audits to provide effective prevention of injury.   Condition noted on or about 07/25/2025.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a.)666 Services/600 Federal Boulevard, Carteret, NJ 07008/Conveyor on Scan station #4/The employer did not ensure that the bottom portion of the conveyor at Scan Station #4 was properly guarded to protect employees from injury. The machine lacked a guard to prevent employees from coming into contact with moving parts.  Condition noted on or about 07/25/2025.

This condition exposed employees to amputation hazards.    Condition occurred on or about 07/25/2025.
