29 CFR  1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  On or about December 17, 2025 and at times prior, at 726 N Hydraulic Wichita, Kansas, employees engaged in forklift operation activities were exposed to struck-by hazards. The employer had not provided forklift training to employees to ensure safe forklift operations were understood and followed.

29 CFR  1910.178(m)(3):Unauthorized personnel were permitted to ride on industrial truck(s):  On or about December 17, 2025 and at times prior, at 726 N Hydraulic Wichita, Kansas, employees  were exposed to struck by and fall hazards. The employer allowed employees to ride on powered industrial trucks that were not equipped with a designated seat or an approved riding platform.  The forklift was not designed to carry passengers, and no modifications had been made to safely accommodate riders, exposing the employee to the risk of falling or being crushed during movement or tipping.

29 CFR  1910.305(b)(2)(i):Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  On or about December 17, 2025 and at times prior, at 726 N Hydraulic Wichita, Kansas, employees  utilizing the restroom were exposed to electric shock hazards in that the light switch did not have a fully covered plate to prevent contact with the live 120-volt inner conductors and components of the switch.

29 CFR  1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  On or about December 17, 2025 and at times prior, at 726 N Hydraulic Wichita, Kansas, employees  were exposed to shock hazards in the following instances:  a. Employees operating the Rye Corp Husky Gantry Bridge Saw exposed to electrical hazards due to no strain relief being provided for the 220v electrical control box exposing inner conductors.   b. Employees operating the Rye Corp Fab King Rotary Cutter exposed to electrical hazards due to no strain relief being provided for the 220v electrical control box exposing inner conductors.
