29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a) On or about December 16, 2025, at a new residential construction site located at 5018 Stefan Ridge Way, Buford, GA 30519, employees performing roofing work on a 9/12 pitched roof were exposed to fall hazards of approximately 18 feet to the ground level. The employer did not ensure that employees were provided with and used a full-body harness with the attachment point located at the center of the back near shoulder level, or above the head, as required. Instead, employees were using positioning belts, which do not meet the criteria for a personal fall arrest system, while engaged in roofing activities.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) New residential construction located at 5018 Stefan Ridge Way, Buford, GA, 30519, Roof: On or about December 16, 2025, the employer exposed employees to fall hazards of approximately 17 feet to the lower ground level, in that the ladder used to access the roof of the structure was not extended at least three feet above the upper landing surface or secured to prevent accidental movement during use.
