29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about December 17, 2025, at a jobsite located at 261 Main Street, Belgium, WI 53004, employees conducting roofing activities (roof repair and shingle installation) on a 5:12 pitch roof of the residential structure were not protected from falling to the lower lever. This exposed employees to a fall hazard from the roof of a residential home of approximately 8 feet.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) On or about December 17, 2025, at a jobsite located at 261 Main Street, Belgium, WI 53004, the employer did not ensure that employees exposed to potential fall hazards had been properly trained to recognize, minimize, or eliminate the fall hazards associated with this work. This exposed employees to a fall hazard approximately 8 feet onto grass, building materials, and concrete.
