29 CFR 1926.451(a)(6): Scaffolds were not loaded in accordance with the design by a qualified person: (a) On December 12, 2026, at 102 2nd Avenue South in Fargo, North Dakota, in the east stairwell, an employee was installing insulation while standing on a scaffold constructed from an A-frame ladder and an extendable plank stretched over an open stairwell. The employee weighed approximately 390lbs was working from the extendable plank where the maximum load was 250lbs.
