(a) Worksite - Scaffold: On or about 12/18/2025, an employee was exposed to fall hazards while performing chimney work approximately 17 feet above the lower level without any means of fall protection.

(a)  Worksite - Roof: On or about 12/18/2025, an employee was exposed to fall hazards while performing chimney work approximately 17 feet above the lower level without any means of fall protection.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  (a) Worksite: On or about 12/18/2025, the employer did not prepare a written fall protection training certification record for an employee who was exposed to fall hazards while working from a residential roof.
