29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  A) On or about December 18, 2025, the employer required employees to perform servicing or maintenance to include, but not limited to, removing and installing grinding wheels on multiple surface grinders where the unexpected energizing, startup or release of stored energy could occur and cause injury. The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure the machine(s) or equipment were isolated from the energy source(s) and rendered inoperative.  B) On or about December 18, 2025, the employer required an employee to perform servicing or maintenance to include, but not limited to, performing setup and cleaning operations on a Yama Seiki VP-2012 vertical milling machine where the unexpected energizing, startup or release of stored energy could occur and cause injury. The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure the machine(s) or equipment were isolated from the energy source(s) and rendered inoperative.  C) On or about December 18, 2025, the employer required an employee to perform servicing or maintenance to include, but not limited to, performing setup and cleaning operations on an OKK VM7III vertical milling machine where the unexpected energizing, startup or release of stored energy could occur and cause injury. The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure the machine(s) or equipment were isolated from the energy source(s) and rendered inoperative.

29 CFR  1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures:  On or about December 18, 2026, two employees from and outside employer were performing maintenance on an OKK MCV1060 Vertical Machining Center and the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures.
