29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:   At the Facility Maintenance Shop, on or around December 11, 2025, and at times before that date, the employer did not ensure that the oxygen and acetylene compressed gas cylinders were separated by a minimum distance of 20 feet, or by a noncombustible barrier at least 5 ft. high having a fire resistance rating of at least one hour. Employees were potentially exposed to fire hazards.

29 CFR 1910.303(b)(2): Listed or�labeled electrical�equipment was�not used�or installed�in accordance�with instructions�included in�the listing�or labeling.  Located inside the Bark Beetle Research building, on or about December 11, 2025, and at times prior thereto the employer did not ensure that a relocatable power tap was being used or installed in accordance with instructions included in the listing or labeling. A freezer (Thermo Scientific, model GT20LFEET8A) was energized by the relocatable power tap. Employees were exposed to burns from potential fire hazards.
