29 CFR 1926.760(a)(1):Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about December 15, 2025 at a new commercial building construction work site, an employee conducting steel erection activities more than fifteen feet above a lower level was not in use of a fall arrest system.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761: On or about December 15, 2025, and at times prior thereto, at a new commercial construction site, three employees performing metal roofing work were not provided fall protection training. The employees were exposed to a fall hazard of more than 15 feet.
