29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: A) 1640 New Market Ave, South Plainfield NJ The lockout/tagout training provided by the employer was not adequate in that employees did not utilize lockout procedures, which are a required part of the training, to de?energize the plasma burn table before retrieving a piece of steel from the table. This condition exposed employees to amputation hazards. Condition occurred on or about 10/24/25.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A- 1640 New Market Ave, South Plainfield NJ 07080 1- The employer did not ensure that the plasma machine was equipped with proper guarding between the table and the gantry track, thereby failing to prevent employees' feet from being exposed to a caught?in?between hazard. 2- The plasma machine was not provided with any type of machine?guarding method to prevent employees from being exposed to moving parts during operation. The machine travels the entire length of the table, and there was no barrier, guard, or other protective measure to prevent anyone from entering the area while the machine was in motion. This condition exposed employees to struck by hazards. Condition occurred on or about 10/24/25.
