29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: 189 First St., Pittsfield, MA: On or about December 22, 2025, the employer did not ensure that subcontracted employees using personal fall arrest systems had their lanyards attached to their harness at a point in the center of wearer's back, while working at heights of approximately 25 feet during a residential construction project involving shingle replacement.
