29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On or about December 22, 2025, employees in the process of conducting roofing activities on a residential roof located at 1356 Kalamath St., Denver, CO 80204, with a pitch of approximately 8-in-12, were exposed to fall hazards of approximately 10 feet. The employer failed to ensure employees were utilizing fall arrest systems and/or devices.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:  a) On or about December 22, 2025, the employer failed to ensure that employees completing roofing activities on a residential roof located at 1356 Kalamath St., Denver, CO 80204, were trained in the appropriate use of fall protection. Equipment includes, but is not limited to, personal fall arrest systems or devices.
