29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system: On or about December 19, 2025 , Employees were exposed to fall hazards of approximately 20 feet while engaged in residential construction activities. Fall protection was not being utilized while employees were performing roofing work. No abatement certification or documentation is required for this item.

29 CFR 1926.502(d)(17):The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. On or about December 19, 2025, the employer did not ensure that employees connected their harnesses to fall protection anchorage points correctly. Employees performing work at heights in approximately 20 feet had their lanyards attached to the front side of their harnesses, exposing them to serious injury in the event of a fall. No abatement certification or documentation is required for this item.
