On or about December 23, 2025, at the jobsite located at 557 N Harbor City Blvd. Melbourne, FL: employees were exposed to fire hazards while dispensing  unsaturated polyester resin Liquids which is Category 3 flammable liquid, in that, the nozzle and container were not electrically interconnected during the transfer process of Polyester Resin GP 500.

29 CFR  1910.1200(e)(1):The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)  a. On or about December 23, 2025, at the jobsite located at 557 N Harbor City Blvd. Melbourne, FL:  the employer did not develop, implement, and maintain at the workplace a written hazard communication program and explain the possible risks to chemical as extremely flammable liquid and vapor, can cause genetic defects, skin and eye irritation and causing cancer. Employees were observed performing daily tasks such as, but not limited to, like transferring process of Polyester Resin GP 500.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   a. On or about December 23, 2025, at the jobsite located at 557 N Harbor City Blvd. Melbourne, FL: the employer exposed employees to severe skin burns, serious eye damage, and vision loss, in that the employer allowed employees transferring chemicals including but not limited to Polyester Resins, BRP XD50, ECSTAR 10W-AP among others without providing employees with effective information and training on the hazardous chemicals in their workplace.

29 CFR  1910.134(k)(6):The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  a. On or about December 23, 2025, at the jobsite located at 557 N Harbor City Blvd. Melbourne, FL:  the employer exposed employees to respiratory hazards, in that employees voluntarily wore the Particular Respirator 3M 1/2 face elastomeric with combined cartridge VOC/ Acid / P100  while performing Polyester Resin transfer and related warehouse tasks and were not trained on the basic advisory information contained in Appendix  D.
