29 CFR 1904.40(a): The employer did not provide an authorized government representative requested injury and illness records within four business hours. 431 Minor Street, Bristol, CT 06010: On or about, January 13, 2026, Employer did not provide the OSHA Form 300, 300A, & 301, records for years 2023, 2024, & 2025, within 4-hours after initial request from a government representative. On January 13, 2026, a Department of Labor (DOL) Occupational Safety & Health Administration (OSHA) Compliance Safety & Health Officer conducting an Employee Injury investigation under the OSH Act of 1970 requested the Employer's OSHA 300 records. Employer provided redacted OSHA Form 300, 300A, & 301 records for years 2023, 2024, & 2025 on January 20, 2026.
