29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: At a commercial worksite located at 4320 W Kearney St, Springfield MO -- a) LP cylinders were stored outside of an authorized storage container, b) LP cylinders were stored near highly flammable cabinets, and c) LP cylinders were stored in a location where heavy truck traffic (garage door) for shipping/receiving. These exposed employees to fire, smoke, and struck by hazards. These were documented on or about 12/18/2025.

29 CFR 1910.178(m)(1): Trucks shall not be driven up to anyone standing in front of a bench or other fixed object. At a commercial worksite located at 4320 W Kearney St, Springfield MO -- an employee was completing an order at the Paka Station when a Turret Truck approached the employee. The mast end of the truck caught onto a cart full of orders, twisting it into the station causing the employee to be pinned between the truck's mast and the cart. This exposed the employee to struck by and caught in between hazards. This was documented on or about 12/18/2025.
