The employer must ensure that each walking-working surface can support the maximum intended load for that surface. a)On or about December 20, 2025, the employer failed to ensure that the interstitial ceiling panels used as a walking-working surface could support the maximum intended load. An employee without knowledge of the load rating was in the interstitial space observing contractors performing work that involved cutting through a drop ceiling comprised of Mesa insulated metal panels.

29 CFR 1910.22(d)(3): When any correction or repair involves the structural integrity of the walking-working surface, a qualified person performs or supervises the correction or repair. a)On our about December 20, 2025, the employer did not ensure that a qualified person performed or supervised repair work affecting the structural integrity of an interstitial walking working surface. Employees, performing work activities, including installing an HVAC motor in the interstitial space above the drop ceiling, which required the opening and repairing of the walking working surface were exposed to a fall hazard of approximately 20 feet to the production floor below.

29 CFR 1910.28(b)(3)(i): Holes. The employer must ensure: (i) Each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: (A)Covers; (B)Guardrail systems; (C)Travel restraint systems; or (D)Personal fall arrest systems. a)On or about December 20, 2025, the employer failed to ensure employees were protected from falling through an opening in the ceiling approximately 20 feet above the production floor while performing millwright activities, including hoisting and installing a motor. Employees were standing near and working around a 12-foot section of ceiling panel that had been removed without any cover, guardrail system, or properly anchored fall protection. As a result, employees were exposed to a fall hazard of approximately 20 feet.
