OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:  a) Summit Resource International (SRI), Steel Storage Racks: On or about December 19, 2025 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading product from the steel storage racks that were not securely fastened to the concrete pad.  b)Summit Resource International (SRI), Steel Storage Racks: On or about December 19, 2025 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading product from the steel storage racks that did not display a plaque designating load sizes.  The employer failed to identify the load rating of the storage racks to ensure they were not overloaded.  c) Summit Resource International (SRI), Steel Storage Racks:  On or about December 19, 2025 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading product from the steel storage racks that contained damage.  The employer failed to identify and remove the damaged steel storage racking components.
