On or about and at times prior to December 23, 2025, at the worksite located at 3006 Stone Fence Place, Green Bay, Wisconsin; the employer did not assess the exposure of employees who are or may reasonably be expected to be exposed to respirable crystalline silica at or above the action level, exposing employees to the hazards associated with respirable crystalline silica in the following instances:  a) When employees were cutting limestone with a handheld cut-off saw. The material safety data sheet (MSDS) stated that the limestone contained approximately >1% crystalline silica. b) When an employee emptied a hopper containing a Portland Lime Mortar. One bulk sample of Portland Lime Mortar contained 10% crystalline silica.
