29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On October 20, 2025, a belly-dump truck backed over an employee causing multiple fractures which resulted in a hospitalization. The hospitalization was not reported within 24-hours to the local Area Office of the Occupational Safety and Health Administration.
