29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    A) 943 N. Division Street - Peekskill, NY: Employees were exposed to fall hazards of approximately 14-feet while working on a roof without the use of a personal fall arrest, or other fall protection systems, on or about, December 8th, 2025.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A) 943 N. Division Street - Peekskill, NY: Employees were exposed to fall hazards of approximately 11-feet while using an extension ladder to access a roof. The extension ladder that was used, was only extended approximately 1-foot above the landing and not the required 3-feet, on or about, December 8th, 2025.
